How do I get started in this program?

  • Adults – Complete Intake process at Battlecreek location
  • Children and Adolescents – Complete Intake process at Flint River Center
  • What to bring — All individuals must provide proper identification, such as Proof of Residency, Names of previous treatment providers, names and doses of medications (current and previous), Tax returns or Award Letter from DFCS

 

Who uses this program?

Adult Men, Adult Women or Children and Adolescents with a Primary Mental Health or Substance Abuse Diagnosis and at least one of the following:

 

    • Recently out of Jail
    • Recently out of the Hospital
    • Homeless or at risk of homelessness
    • Need help obtaining food stamps, SSI, TANF, and/or Medicaid or employment skills
    • Need assistance with symptoms and medication
    • Need assistance with Daily Living Skills

 

When are services provided?

  • Monday – Thursday  8am-7pm
  • Fridays 8am- 5pm

 

What services are provided?

  • Applying for Social Security, Medicaid, Food Stamps
  • Assistance with Applications for Educational Activities such as GED, HS Diploma, College
  • Assistance with Employment Resources & Job Retention Skills
  • Assistance with Dealing with your symptoms & medications
  • Assistance with problems with your family, friends, or other loved ones
  • Assistance with Crisis and Safety Planning: What to do when you are in trouble
  • Assistance with getting and staying out of jail, probation, or parole
  • Assistance with Housing and Arranging for Transportation
  • Assistance with Daily Living Skills

 

What is the intended goal of this program? What does this program do?

  • The Goal of Community Support Services is to assist Individuals in not only stabilizing but becoming functioning members of the community. We strive to support Individuals in living independently and the successful self-management of symptoms and behaviors

 

How are services structured (group setting, 1:1, in home, etc)?

  • Community Support Services are provided 1:1 only
  • Community Support Services are provided in the home OR any community setting based on Individual’s preference
