Residential Care Services:
Developmental Support Services Developmental Support Services Team is the first point of contact for all consumers, age three and above, requesting MR/DD services. Information and referral/linkage to community resources, intake, diagnostic assessment and individualized program planning, crisis intervention, behavioral support services, family support services, family training/counseling, individual counseling, psychological evaluation, nursing assessment, physician assessment, and professional training are primary services provided by this team. Services are provided Monday through Friday from 8:00 a.m. to 5:00 p.m. After hours appointments are available under extenuating circumstances. Private pay fees are based upon the consumer's income and family size. Medicaid, Medicare and Peach State are accepted, and some private insurance is accepted with prior approval. There may be grant in aid dollars available to support services. Services are provided at 7146 Southlake Parkway, Morrow, GA 30260. Clayton Center MH Residential Services are divided into two components: the Group Residences (State Licensed Personal Care Homes) and the Supportive Living Apartments (SLA's). Group Residences provide community living arrangements that support you for either a long or short period of time based on your identified needs. These residences are licensed state personal care homes that provide 24-hour supervision in small group home settings located in typical neighborhoods and not distinguishable from other homes in that area. There are usually no more than 2-5 unrelated consumers living in each home. In Group Residences, you will be offered a minimum of (6) hours of skills training weekly. This training is provided in order to help you enhance and/or develop skills needed to improve your life functioning. Your specific needs will be determined by an assessment and placed in your individual treatment plan, commonly referred to as the "MICPS." The treatment team usually consists of your caseworker, psychiatrist, and a representative from each of Clayton Center's service programs where you receive services. This skill building and/or developing is designed to help you progress toward a less restrictive MH residential program option, usually within 6-36 months. The length of stay in a Group Residence is dependent upon whether or not you have developed and/or properly used the skills needed to move on to a less restrictive environment such as Supported Living, Independent Living, or a private personal care home in the community. The Supported Living Program provides many of the same services as the Group Residences, but also provides an opportunity for you to practice the skills you have developed, restored, or improved while in the Group Residences. To have an SLA you must either have to be employed or participate in a day program. Staff support will vary from several hours per week to several hours per month. SLA's provide individualized levels of support designed to foster growth, improvement, and comfort with being independent. Under this component of the MH Residential Program you will increase your level of independence, so that you may progress to Independent Living or another housing option of your choice, usually between 12-24 months after admission. In some instances, you may be required to move from Supported Living to a Group Residence due to an increase in symptoms of mental illness or inadequate independent living skills. The Clayton Center MR Residential Program provides community housing, skills training, and related services to severe and persistently developmentally disabled adults residing in Clayton County. These services are designed to be rehabilitative in nature and accommodate/support normative life patterns. It is the goal of the MR Residential Program to provide needed services which improve, restore, and/or help consumers develop skills in particular areas that interfere with their ability to live independently in the community and be productive, law-abiding citizens. The MR Residential Staff provides instruction, guidance, and support to each consumer based on his/her individual needs, but all consumers are encouraged to move towards independence and integration into the community through chosen roles and environments. Clayton Center MR Residential Program provides equal access to its program services for severe and persistently developmentally disabled adults without regard to race, color, creed, sex, age, religion, national origin, sexual orientation, or disability. Residential services provides an array of living arrangements in homelike settings to meet the needs of persons served despite the length or severity of their disability. Accessible services are developed and implemented to all persons with Developmental Disabilities by providing sufficient flexibility to meet the changing needs of each individual. Clayton Center MR Residential Services are divided into several components: Group Residences (State Licensed Personal Care Homes), Supportive Living (SL), In Home Supports and Host Homes. Residential homes are scattered throughout the community in various neighborhoods next to non-disabled neighbors, and close to neighborhood merchants, maximizing community integration. The particular type of residence depends upon the needs of the consumer. Residential services may be highly structured, heavily supervised, and programmatically intense; or services may facilitate a relatively independent lifestyle requiring only a modest amount of staff support. Holly Hill Respite Respite is defined as a 'brief interval of rest'. The Holly Hill home provides a home-like atmosphere and safe place for guests with developmental disabilities to stay while their families are enjoying an evening out, a weekend away, or a two-week vacation. The house is designed to accommodate four overnight guests in private bedrooms while sharing the use of a bathroom with another guest. The home is completely accessible for its guests. Encouraging guests to bring their favorite items from home enhances a home-like atmosphere. Food preferences are respected whenever possible and favorite activities are included in the routine. Respite services are available to families and relatives of individuals with developmental disabilities, specifically people with complex needs. Individuals who have limited self-help skills, complex medical needs are encouraged to use services. The home is not suited to people who: 1) already have respite available to them; 2) to people who need nursing care or need to recuperate from medical treatment; 3) to people who need stabilization; or 4) to people who are dangerous to themselves or others. The Clayton Center Respite Program provides a brief period of support or relief for caregivers or individuals with disabilities. Respite is provided: 1) when a consumer is experiencing a crisis and needs short term support; 2) when families or the usual caretakers are in need of additional support or relief; or 3) when the consumer needs relief or a break from the caretaker. Services can be provided during the day, as well as overnight. |
